This form provides members of the Fanshawe College community with an opportunity to disclose incidents of harassment, discrimination, or oppression, whether experienced directly or witnessed.
Completing this form does not automatically initiate a formal complaint, and no investigatory action will be taken in response to the information disclosed in this form. Our human rights policies safeguard procedural fairness and govern the investigation process.
This form is intended only to support informal reporting and track incidents at an aggregate level (i.e., high-level, summary statistics that describe a trend or group) to better understand patterns at the College and inform institutional responses and strategies.
What happens when I submit the form?
- Disclosures that provide your contact information and consent to be contacted, will receive a follow-up from the Centre for Equity, Diversity and Inclusion. The Centre for EDI will offer support and referrals to available avenues for formal complaints and resources. Making a disclosure does not mean you are filing a formal complaint.
- Anonymous reports will be directed to appropriate support resources but will not receive follow-up contact.
Unless the limits to confidentiality (discussed below) apply, only staff in the Centre for Equity, Diversity and Inclusion will review your responses.
Will my information be kept anonymous?
All disclosures will be seen only by designated staff in the Centre for EDI. We are committed to maintaining anonymity, with a few exceptions:
- Where there is a risk of harm to yourself or others.
- Where there are legal or professional ethical obligations to flag your concerns, such as reports of child abuse.
- Where CEDI or the College is legally compelled to disclose information.
- Other exceptions as required by law or college policy.
How will the information be used?
Data collected through this form may be used to support:
- Equity-related programming and institutional initiatives;
- Development of education and training;
- Internal reporting on campus climate and patterns of harm;
- Anonymized summaries of incidents for reporting purposes (e.g., "X% of reports relate to racial bias in student services").
All identifiable details will be removed before reporting.
If you provide your name and contact details, that information will be stored securely per the College's data retention and privacy policies and deleted thereafter. De-identified, anonymized data may be retained per the College's data retention and privacy policies to support long-term institutional improvement.
